BONE REGENERATION

In bone regeneration, it is not enough to put in “synthetic bone”. There are very precise rules, established by biology, that must be followed to achieve true bone regeneration. Different materials can be used: autologous bone, taken directly from the patient’s mouth; xenografts, when the strictly certified material comes from another animal. Bone of animal origin, before reaching the mouth, undergoes a long series of chemical and physical steps that ensure that it retains only its mineral component, which is unable to generate immune responses.

Why does one have to wait months before being able to proceed?

Because the newly grafted bone needs time to take root. The bone cells slowly colonise the bone graft, making it become one with the surrounding bone. But that’s not all! Even when integrated, the new bone is not yet able to withstand chewing loads: it must therefore go through a functional education phase, whereby the bone trabeculae prepare themselves to resist chewing. The greater the lack of bone, the longer it has to wait – these are the laws of biology.

ZIRCONIA IMPLANTS

Zirconia implants are dental implants, similar to traditional titanium implants, but made from a different material: zirconia.
Zirconia is a ceramic material, a polycrystalline ceramic. Titanium remains the material of first choice for dental implants and most of the scientific literature is written about titanium implants. However, zirconia is even more biocompatible than titanium. These ceramic implants are required by patients who have hypersensitivity to titanium or who prefer metal-free solutions.

Are zirconia implants as good as titanium ones?

Yes, they are a safe and tested solution; however, they have fewer years of research behind them than titanium, which is why we propose them when specific requests are made to us. We believe that zirconia implants are a very valid alternative to titanium implants and we are members of the European Society of Ceramic Implantology, the most important scientific society in Europe for this type of implantology.

COMPLEX CASES

By complex cases we mean situations where the diagnosis requires a more detailed level of diagnostic investigation, when “finding caries” is not enough. Complex cases often require a multidisciplinary approach.

How complex cases are treated

But when the dentition is very compromised, is it not advisable to remove all the teeth and make implants?

It depends mainly on the clinical situation; our first task is to save the teeth. It often happens that all teeth are extracted in order to insert implants: in the short term it may seem a winning choice but it is not always so. If I remove my teeth at the age of 45, I have to hope that the implants will last about 40 years. However, implants are prostheses, like a hip replacement, and only a few have such a long view.

After a thorough diagnosis, if it is possible to save the teeth, it is better to do so, although the treatment will take more months than extracting all the teeth for immediate loading. Only non-recoverable teeth will be extracted and the implants necessary to restore optimal chewing will be inserted, creating an implant-tooth synergy that will prolong their life.

Once the treatment is over, periodic check-ups are carried out as recommended. Committing time, effort and money to a complex treatment and then not showing up for maintenance appointments is a guarantee of failure. It is like renovating your house and then stopping cleaning and maintaining it.

Scientific studies show that dental problems, such as caries, gingivitis and malocclusion, can directly affect performance quality, recovery and even physical endurance.

The Role of Dental Occlusion

Your mouth is more important than you think. Correct dental occlusion is not just for eating or smiling: it is essential for maintaining postural balance and reducing the risk of muscular pain. Athletes with malocclusions or bruxism often report tension in the neck, back and temporomandibular joints. These problems can affect physical stability and reduce the ability to recover during rest.

The impact of oral health on performance

  • Caries and periodontitis: untreated problems can cause systemic inflammation, which increases muscle fatigue and slows down recovery.

  • Dehydration and salivary flow: intense physical activity and mouth breathing can reduce saliva, increasing the risk of dental caries and erosion.
  • Sports drinks and sugars: frequent use of energy drinks and carbohydrate-rich snacks, common in athletes, can worsen oral health, leading to dental erosions and caries.

Practical solutions

Although the problems are real, there are simple and practical solutions to keep your oral health at the highest level:

  • Regular check-ups: visits to the dentist at least twice a year to monitor occlusion and prevent major problems.
  • Personalised hygiene: the use of high fluoride toothpastes and specific mouthwashes can reduce tooth wear and improve gum health.
  • Sugar management: replacing sugary sports drinks with alternatives after training can make a difference.
  • Customised Bite / Mouthguards: when needed, they protect teeth during sporting activity and improve jaw stability.
